it. In order to provide our customers with a broad range of products and services as effectively and conveniently as possible, we use
technology to manage and maintain customer information.
The following policy serves as a standard for all Heartland Insurance & Realty, Inc. employees for collection,
use, retention, and security of nonpublic personal information.

What Information We Collect

We may collect "nonpublic personal information" about you from the following sources:
- Information we receive from you on applications or other account forms;
- Information about your transactions with us, our affiliates or others; and
- Information we receive from third parties such as credit bureaus, medical providers, motor vehicle record agencies, or others.
"Nonpublic personal information" is nonpublic information about you that we obtain in connection with providing a product or service
to you. For example, nonpublic personal information includes information regarding your policy and payment history.

What Information We Disclose

We are permitted under law to share information about our experiences or transactions with you or your account (such as your account balance and your
payment history with us) with companies related to us by common control ownership (“affiliates”). We also may share additional information about you
or your account (such as information we receive from you in applications and information from credit reporting agencies) with our affiliates. You may
direct us not to disclose to our affiliates information that does not relate solely to our or our affiliates’ experiences or transactions with you or
your account (such as the application information and credit bureau information) by calling us at 1-800-626-5213.
We are also permitted under law to disclose nonpublic personal information about you to “nonaffiliated third parties” (i.e., third parties that are not
members of our corporate family) in certain circumstances. For example, we may disclose nonpublic personal information about you to such third parties
to assist us in servicing your account with us; to government entities in response to subpoenas; and to credit bureaus. We do not disclose any
nonpublic personal information about you to any other third parties, except as permitted by law.

We may also disclose the following information to companies that perform marketing services on our behalf or to other institutions with
which we have joint marketing arrangements:
- Information we receive from you on applications or other forms, such as your name, address, social security number, family member information, assets, income, and property locations and values;
- Information about transactions with us, our affiliates or others, such as your account balance, policy coverage, payment history, the premium you pay, preferences, claims information, the method of your purchase, and parties to transactions; and
- Information we receive from credit bureaus, such as your creditworthiness, motor vehicle and driver data, medical and employment information, loss history reports, additional driver data, and your payment history.
If you decide to close your account(s) or become an inactive customer, we will continue to adhere to the privacy policies and practices described in this notice.

Medical Information

It may be necessary to collect medical information to provide you a product or to pay a claim. We do
not use or share it, internally or externally, for any purpose except the following:
- Underwriting insurance
- Administering your policy, account or claim;
- As required or permitted by law; or
- As otherwise authorized by you.

Visitors to Heartland Insurance & Realty, Inc. website remain anonymous. Heartland Realty & Insurance, Inc. does not collect identifying information about visitors to our site. Visitors may elect to provide
us with personal information via email. This information, if provided, is used internally as appropriate to handle the sender’s request. It is not disseminated or sold
to other organizations or businesses.
